Role Summary
Band 8a Advanced Practitioner, or Band 7 Trainee Advanced Practitioner to work across our Acute Hospital at Home, Frailty and Hospital Outreach services in North Devon.
The role will require working in a variety of settings including the acute trust sites and patients own homes, in order to best support patients to remain safe and well at home, supporting alternatives to admission.

We are looking to recruit highly enthusiastic and motivated qualified Advanced Practitioners (AP) or trainee APs to be part of our Hospital Outreach, Acute Hospital at Home and Acute Frailty Services in North Devon. The successful candidates will work in accordance with the Trust frame work for Advanced Practice or be willing to undertake the training programme to achieve AP status.
The AP role will be integral in driving forwards our services and in the provision and delivery of acute, patient centred and holistic care. You will have the opportunity to work in a variety of settings including the Community, Emergency Department, Medical Assessment Unit, Same Day Emergency Care Unit and on the wards.
We encourage applications from Nursing and Allied Health Professionals who are willing to work generically across traditional professional boundaries to provide holistic best practice care for patients with complex needs.
As we move towards a new era of integrated care and working collaboratively across organisational boundaries, the need for dynamic, focused teamwork is vital and the AP will be an integral member of services. The AP will be key in building relationships across acute and community settings and will be provided with a wealth of learning opportunities.
Trainee APs will receive funded training, support and protected non-clinical time to undertake their studies. The AP course is a master’s level qualification so a clear commitment to learning is vital for those seeking the trainee AP role. You will be paid at Band 7 level until you achieve your AP qualification.
As you will be working in the community as well as the acute hospital you will need to hold a valid driver’s licence and have access to your own transport.
Pay: Please note, the trainee position will be paid Band 7 until they meet the required competencies
Working Pattern: This role is 37.5 hours per week Monday – Friday with a working pattern to be discussed.
