Role Summary

  • The post holder will work as across a range of specialty Acute Medicine functions, primarily this will be Acute Hospital at Home (AHAH) and Outpatient Parenteral Antimicrobial Therapy (OPAT) but will include involvement with Same Day Emergency Care (SDEC)
  • Within the AHAH role they will lead in the delivery of a seamless transition between the acute trust and the community, ensuring patients are kept safe and the best possible care is delivered. Current patient streams managed by this service are OPAT, AF, COVID, AKI, Heart Failure, NSTEMI, Syncope and Frailty. We will continue to introduce new work streams.
  • They will proactively seek new patients to recruit from MAU, SDEC and wards across the hospital, assessing these patients and driving their safe discharge into AHAH.
  • They will work to improve and maintain the AHAH service based on best practice and current national guidelines.
  • The post holder will be expected to complete ongoing learning and obtain an advanced practice module relevant to their role, including history taking and clinical examination. 
  • To provide on-going professional and clinical leadership to the care team and deliver evidence-based care as part of the wider multidisciplinary team.
  • The post holder is expected to carry out all relevant forms of care and procedures, for which they are competent to practice, without direct supervision.

Are you looking to be part of something new and exciting? If so, we have a fantastic opportunity for a Registered Nurse to bring their experience and knowledge to our integrated Virtual Ward at our Northern site. 

The role of Acute Medicine Specialist Nurse has been developed as a response to the growing needs of the local community to have a diverse multidisciplinary team. This role will include working in a range of areas and modalities. With the opportunity to complete an advanced practice module after an appropriate amount of time in post.

Virtual ward is a safe and efficient alternative to NHS bedded care. Virtual wards support patients who would otherwise be in hospital to receive the acute care and treatment they need in their own home. This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital.

The delivery of digitally enabled care (virtual care) including virtual wards is a national priority embedded within current health and social care policy.

This post, as part of a multi-disciplinary team, provides an opportunity to drive and transform the system by embedding the use of digital technology to release pressure on acute care and improve patient outcomes.

Working Pattern: 15 hours per week. You will be expected to work days including weekends and bank holidays.
